Athletic Commitment Form

    Use this form if you are MAILING YOUR COMMITMENT PAYMENT.
    This from will serve as your commitment until your mailed payment arrives.

    ​Do not use this form if you are making your payment through the website. Making payment online serves as your official commitment.   

    Instructions
    Please check the boxes for only the sports that your are making payment for at this time.  Commitment fee payments must be received by the commitment deadline for each sport.  Teams will not be put on the schedule if payment is not received by the due date.  If your organization prefers to pay by check or must secure a purchase order before a check can be issued, please plan ahead so that your payment will be received by the due date.

    Information Update

    Please confirm that your coach's contact information and home game location is up-to-date on the your organization's page under the members' section of the ACEL website for all sports checked above.  To change or update information for your organization, fill out the Information Update form located on the website. 

    For Basketball and Volleyball only

    Basketball and Volleyball teams must submit a completed gym diagram (downloaded from the website) for their home gym.  Gym diagrams must be submitted for every gym in which you regularly host home games.  Gym diagrams are due by the commitment deadline for each sport.  Teams will not be put on the ACEL schedule without submitting a gym diagram.  
    Please be aware:
    • Commitment fees are not refundable after the commitment deadline has passed.  
    • There is a $500 fine for withdrawing from a sport after the commitment deadline.
